| This is a .45 FTE Physiotherapist 2 position working with a peer. The days of work are Thursday and Friday (some flexibility if needed)The position assesses,plans,delivers,evaluates and provides care coordination to meet the needs of specific children (5 - 19) and their families experiencing physical,neurological and developmental challenges and those requiring episodic or palliative care. Using a multidisciplinary team approach the physiotherapist collaboratively addresses the client's functional/physical/social/cognitive/perceptual/environmentalneeds to maximize capabilities, community participation, recovery from or adaptation to illness/impairment and achieve excellence in health services and education.Services include identifying client's needs/desired functional status,quality of life and strengths and weaknesses, and through analyses develops/negotiates an evidence based outcome focused care plan including short and long term goals to achieve optimal health outcomes and/or improved functional independence and/or physical performance strategies. In accordance with established treatment guidelines identifies available donation sources and/or requests funding for equipment. The physiotherapist contributes to the organization and the delivery of physiotherapy services by participating in program qulaity assurance intiatives meeting, research and education. |
